CVUSD Health Center

 

The Chino Valley Unified School District (CVUSD) Health Center is a California-Based Health Center (CSBHC) that serves children and families within the CVUSD and surrounding community. The goal of our program is to improve health outcomes by increasing student access to quality and comprehensive health care. Our program strives to enhance access to health care by bringing it directly to where students and families are and conducting active school-based outreach to connect students with care. We provide intensive support for the highest need students by being present on a daily basis to manage chronic disease, address behavioral health issues, deal with crises, and help students and families access resources. We provide support for the school district’s mission to improve academic achievement by working together to address absenteeism and addressing health concerns that may pose as barriers to learning. The CVUSD Health Center provides the following services with the ultimate goal of keeping kids healthy and ready to learn.

  • Well-child check-ups and CHDP exams

  • School-entry physical exams

  • Sports physicals

  • Sick care treatment and follow –up

  • Immunizations for children ages 0-18 that are uninsured or have Medi-Cal or other Medi-Cal funded plans (i.e. IEHP, Molina)

  • Developmental screenings

  • Hearing and vision screening

  • Specialty referrals as necessary

  • WIC referrals for children under five years of age

  • Guidance with Covered California/Medi-Cal and other health insurance programs

  • Instant on-site temporary Medi-Cal for qualifying children


We are located at the CVUSD Health Center, 12970 Third Street, Chino, on the Adult School Campus. The Health Center is open Monday through Friday, 7:30 a.m. to 3:00 p.m. The Health Center is closed between 12:00 and 1:00 p.m., and has a bilingual staff available to help.
